About Botox®

At our practice in New York City, Botox is one of the most frequently performed procedures. Botox Cosmetic is the #1 treatment for dynamic wrinkles around the eyes (crow’s feet), between the eyebrows (the “11’s or worry lines) and the forehead. Other treatment areas include the masseter or jawline muscle, the upper lip, the nose, and even the armpits to reduce sweating.

Botox Cosmetic is the most popular Brand. Other brands include Dysport and Xeomin. At Shafer Clinic Fifth Avenue, we stock all three brands and will discuss with you which one is best for your treatment.

Since its introduction in the 1990s, the uses of Botox have expanded rapidly. However, at the same time, the number of people injecting Botox has risen exponentially.

It is essential to clarify who will be injecting your Botox.

While an understanding of the underlying anatomy is required to achieve excellent results, many clinics and spas have technicians with little or no formal training injecting Botox for their patients.

For 2009, Dr. David Shafer wrote an important chapter and produced an instructional DVD on Botox injections. Starting with his plastic surgery training at the world famous Mayo Clinic and continuing at the prestigious Manhattan Eye, Ear & Throat Hospital, Dr. Shafer stands on the forefront of facial aesthetics.

Dr. Shafer is a MASTER trainer in Botox – this means that he trains other doctors. He has traveled the country and even Japan to teach other doctors his techniques.

It is important for you to know how much Botox is being injected.

Most clinics and spas sell Botox by “area.” However, this allows them to significantly dilute the Botox for a higher profit at the expense of inferior results. It also increases the chance of cross-contamination between patients. Additionally, the clinics and spas may store the diluted Botox for long periods which decreases its effect.

Most patient use an average of 50 Units. 10 units by each eye, 20 units between the eyebrows and 10 units on the forehead. However, your treatment is customized to your specific wrinkle pattern.

Dr. Shafer only uses genuine Botox Cosmetic from Allergan.

Dr. Shafer injects Botox by unit, so the patient gets exactly what they pay for. He then keeps an electronic record of your injection to use as a reference for further injections.

It is important for you to understand the possible complications of Botox.

The most common but very unlikely complications is temporary swelling (30 minutes) and possible bruising (less than 1%).

Dr. Shafer uses very fine needles with highly concentrated Botox to help avoid these unintended consequences.

Some confusion exists between the use of Botox and the use of facial fillers such as Radiesse, Juvederm & Voluma. Botox decreases muscle contraction which helps reduce dynamic wrinkles – wrinkles caused by movement. Botox is usually most effective around the eyes and forehead. Facial fillers help “fill” volume deficits and smooth out static wrinkles – wrinkles caused by volume loss and gravity. Fillers are generally most useful around the mouth and in the mid face, but can be used to define the jawline and nose as well.

Often, a combination of Botox and facial fillers are required to achieve the best results.

Many of our Botox patients come from out-of-town and see us during their vacations and business meetings. Please call the office or email info@drshafer.com to check on availability of last minute appointments.

RealSelf Q&A with David Shafer, MD

Botox “There are two types of wrinkles: Static and dynamic,” says Dr. David Shafer, a double board-certified plastic surgeon in New York City. “A dynamic wrinkle is caused by the muscle underneath the skin squeezing, like when you raise your eyebrows. If that happens repeatedly, a dynamic wrinkle eventually becomes a static, lasting wrinkle.”

According to Dr. Shafer, it’s in these sorts of cases—where repeated action has caused the wrinkle—that Botox can work preventatively, since it temporarily stops the muscle from contracting. And if a dynamic wrinkle is caught early on, Botox can even prevent the deeper static wrinkle from forming entirely. Yes, really.

Botox To get Botox, or not to get Botox—that seems to be the question on the minds of women (and men) spanning from ages 25 to 65. Hailed as the most effective wrinkle treatment on the market today, Botox, or botulinum toxin, is just one of many FDA-approved brands that do the very same thing. “Botox is a brand name just like Kleenex or Xerox, which people associate with the actual product and use as a general term,” says David Shafer, MD, board-certified plastic surgeon and RealSelf contributor. “All of the products work very similarly, but differ in their molecular configurations and manufacturing.

Botox My crash course into the mysterious world of Temporomandibular joint disorder began one fateful fall evening, during my junior year of college. The day started off, as usual, packed with classes and followed by a seven-hour shift at one of my two part-time jobs. What began as a dull ache early in the day, suddenly turned into a searing hot pain that shot through my head as the lights from oncoming traffic flashed across my field of vision during my drive home. It was my first migraine.

Botox Sure, you’re into the latest grooming lotions and potions. But are you ready to make Botox a regular part of your grooming routine? It’s gone more mainstream than you think, and Botox for men may be just the tip of the anti-aging iceberg. According to The American Academy of Facial Plastic and Reconstructive Surgery, the number of men having cosmetic procedures has increased 325 percent since 19. Wow!

In fact, TV analyst, Hall of Famer, and former pro football and baseball player Deion “Primetime” Sanders is now the spokesperson for the Botox brand. “I’ve always taken pride in my appearance. Turning 50 prompted me to take an honest look in the mirror.

Botox A botox needle hardly pricks. The skin it pierces barely bleeds. In less than seven minutes, from slipping into the patient’s chair to departure, you have achieved more than 1,000 jars of La Mer dutifully applied over the course of 12 months ever could.

Had you told me as a teen I would have become a “Botox baby” at a tender 23 years old (shorthand for young adults receiving a treatment generally associated with aging women), I would have been alarmed… and likely impressed. It’s a procedure I would have considered is exclusively available to the affluent, and accompanied by such significant stigma that I could never partake. But Groupon and the like makes cosmetic treatments far more accessible, and our growing obsession with perfection is making them much more appealing. After all, everyone needs a little help, right?
